The main problem is that they have and technically are still fighting a war with them. The potential of threat is terrifying. US forces would destroy their artillery in a blink of an eye if need be. Given the U.S's technological advantage against an attack - they would launch an enormous amount of rockets with great precision and wipe them out. If and it's a big IF - that NK was to follow up on this latest threat - then the US would have no other option but to strike back.Sadly - there will always be innocent people who will perish and /or become casualties in war.
