Depends on the quality of the picture and sound. I've downloaded some with no sound at all and only half the ending, and others with more pixels than Utah and more sound than the M15 during rush hour

It's unlocked second time through the game. Found on a sawn-tree stump right next to Jaspers car in the Forest World.

You can also pickup Eileens nurse costume the second time through in the Apartment World, although it needs to be in the inventory for the whole game from once you get it to actually be used for the third playthrough.

Actually there were TWO Robbie the rabbit cameos in SH4. Both in Eileens room, meaning that she had obviously been to SIlent Hill Lakeside Amusement Part and won a stuffed toy. And also, during the City World, if you look out of the apartment window a Robbie the Rabbit head balloon floats above the buildings.
And, after Eileen is attacked and you're the next on the list, the Robbie doll points directly at you when you peep into her room.

Also: What is the book The Order follow? The Crimson Tomb? Is The Descent of the Holy Mother a seperate book or a part of The Crimson Tomb?

She, much like the rest of the cults leading members, rose through the ranks, mainly by reputation. Of all of the members, she is amongst the most dedicated, and the fact that she was the mother of god kind of ensured her place in the top quarters.

The order 'left' the town when the tourists appeared, as did all of the talk of such things. In truth it never left, just continued behind closed doors. Some would argue that, thanks to the drugs trade with the tourists, it was booming more than ever.
