I dont think we really have much of an idea as to why some of these animals have ended up they way they did. Animals with extra legs, extra heads and other deformailites. Im sure some have myths about them, but maybe its just a freak of nature, same with siamese twins etc.... Or maybe its the pollution and contamination thats causing it all....

Edit: To address the actual post topic, in the case of the frogs at least it was probably pesticides that did it. I recently read a book on the subject called 'A Plague Of Frogs'. During 'outbreaks' in Quebec, Michigan, etc. as many as 30% of leopard frog specimens were found with multiple legs, unusual skeletons, and so on. IIRC, a chemical in pesticide runoff was getting into the frog eggs and messing with the hormone that dictates how many limbs should be grown and where. The hormone was being continually produced as a result of this, and tadpoles formed with as many as 13 limb buds, resulting of course in a 13-legged frog.

Well, with animals with extra legs, heads, et cetera, in the field of extra appendages in general, it's because the embryo tried to split into two or three embryos to make three babies, but didn't complete the split, thereby linking two or three beings together to create something with eight legs when it should have four, three heads, et cetera.
Yeah, it's the same way Conjoined Twins are born.

That cat is famous! I have seen photos of that cat on dozens of websites. My auntie has a little dog that you could shave to look like a 30cm high lion.....

Many of these 'mutations' happen because of us humans interfering with nature i.e. chemicals, experiments etc.
Yes many people over the years have counted that chemicals and experimenting play a big part in mutations but also, like stated before, sometimes it is just nature it self that is too blame. Growing of the ebryo is very important and anything that may go wrong through that process is likely to be noticed straight away.
But many other mutation cases, have not been accioated with these points, there is still the fact that maybe evolution is taking place !
