Not being big headed or anything, but i'm usually pretty good at figuring out the plot twists befor they happen, i.e. Sixth sense, figured it out 20 minutes in. The Village, likewise.
But the only one that caught me out was the end scene of No Way Out with Kevin Kostner ('s**t he was the russian spy all along').
What twist endings have caught you by suprise?

The Village???????? o.k. how could you have known the village was a nature reserve in the year 2003 before it was revealed.
but anyway the usual suspects, Old Boy (OMFG best revenge ending ever just so evil and twisted), Fight Club, Matchstick Men, there are plenty others can't think of more.

It didn't throw me off gard either. About halfway through I turned to Falco Rex and said "they keep all their cell phones in those little boxes".
Usual Suspects had a good twist. It stumped me.
Basic had a pretty good ending and
Skeleton Key surprised me a little as I didn't realize that s.o.b. banker hanged and torched his own kids when he thought he was killing Justify and Cecile.
Suicide Kings actually floored me. Didn't see that coming at all.

To answer the question though, most of the orignial Twilight Zone episodes the first time I saw them. Damn peculiar though, even though I know how they end, I'll still watch re-runs of those shows.
How about the original "Planet of the Apes". Sure, you might have guesses it but, the ending was priceless.
"Fight Club" was good. I never saw that one coming.
Other than that, I really can't think of too many. Most of the BIG movies of the past few decades seem to kind of follow a formula.

The Prestige has several towards the end. They do it in such a way that you figure it out, then change your mind and then they show you that your first assumption was right all along. But there is one that you won't figure out till the very end.

Saw, Usual Suspects, Sixth Sense and Fight Club all spring to mind but what about the mother of all twist endings.... Star Wars.. that will always be the ultimate twist.

Momento had an interesting twist at the end...but that's probably simply because you spent the entire movie wondering what the hell was going on :t Really good idea how to have the plot unfold...

Twist endings are fun
I loved the Unusal suspects one
But yeah i also figured the sixth sense one i wasnt as quick as some though i figured it out during the restraunt scene
the village was a quick one too figured that out at the trailer
the prestige was also probably my slowest i figured it out about ten minutes before the movie gave the answer

I wasn't alive when
Star Wars: The Empire Strikes Back came to the cinemas, but I find it hard to believe anyone in teh audience said "oh yeah, Vader and Luke, they gotta be related". BIggest twist of all time

Other movies - The Game (Michael Douglass and Sean Penn) kept you guessing right to the end - is it a game, is it real? 12 Monkeys had an interesting twist, htough not entirely unsurprising. Fight Club was awesome. Haven't seen a lot of the movies listed so far in this list, but these are my movie twists.

I got the twist for The Village...because they used terms that they wouldn't normaly use in that age. i think they used the word "dollar" in there somewhere. And I thought "What would be the most retarded twist for this movie?".
And the monster being the town leaders was really easy.

Well,twists..I really like'em,specially the ones that can't be guessed from earlier.Taking a look at the movies with twist endings that I've watched;
-The Sixth Sense : Realized what was going on just a min before the revelation.It was a damn creepy movie,and is the masterpiece of M.Night Shyamalan.
-Fight Club : Knew the twist before I've seen it

Nevertheless,the real ending was much more impressive than the twist.
-The Others : Seen it coming from the beginning,kinda easy after seeing Sixth Sense.
-The Game : Totally caught me off-guard,awesome movie with a nice twist at the end!
-Memento : One of the greatest movies I've ever seen,and the ending was just terrific!I sat there on the couch with my jaw open for 15 minutes...
-Identity : I don't think anyone could see it coming,nice movie with a BIG surprise...
Other than these,I've got to mention SW:Empire Strikes Back(which indeed has the biggest twist of all time),Usual Suspects,Planet of the Apes,The Village,Wild Things and Vanilla Sky.

Scorsese totally butchered this, well its more of the writers fault that butchered the original! Anyway its pale compared to the Internal Affairs.

I think you're right, the ending of that movie was very disappointing.
The Green Mile should get a mention, very good twist yet sad but beautiful ending.

I dunno, I didn't like it very much, mostly because I guessed the twist at the beggining when De Niro realizes his wife is dead. I had two theories: "she came back from the grave- it's Hollywood after all- to take revenge because he killed her" and "he's the one, he got crazy and he killed her". Well, at least Famke Jannsen was in it. Good eye candy
As for surprise endings, I know it's not a movie, but many episodes of 24 have got twist endings, including the mother of all twists, the one near the end of Season 1

. Definitely didn't see that one coming.

SAW 1, without a doubt. I've seen dozens of great movies with a twist ending, but none of them have taken me by surprise like Saw did. I was already expecting the ending credits, and then that happened! Woah, I got cold shivers all over my body, and I still do every time I watch that scene. Brilliant, simply brilliant. And the music on the background is just awesome.
When I watched the movie again (and again and again...), I noticed there actually are some clues from which you can kinda figure out the ending, but honestly I didn't pay any attention to them the first time. With some other movies like Se7en, Fight Club, The 6th Sense, the clues were more obvious. But with Saw, I definitely didn't see that one coming!
