This Fire Ball ride below (video from 2013) appears identical to the one involved in the accident, that is, of course, if it isn't the actual one. Anyway, in all the other videos I've found of the ride, only this one below appears it'd have been built during the same time-frame, that it'd be of the same serial number category. So it's an engineering problem then? It seems the way the arm that holds the seats was designed in the 1998 version of the Fire Ball was simply not safe. Because why then KMG (the compagny behind ride) would feel the need to update the Fire Ball in 2010 with a redesigned, more robust arm?
