Want to know how my doctor reached the conclusion I was once depressed, and then put me on anti depressants for quite some time?  He gave me a questionnaire with multiple choice answers!! Took a minute or two to fill out

Then totted up the score and agreed with me, I was depressed !!

Heres a version of it, cant seem to find the actual one I filled out
